I'm a healthcare professional trained in "reasonably large city", but who then did a 4 month practicum in one of the small towns, and then got sent to most of the rest of them for a week or two at a time for my first year and a half in practice. You think you know what it will be like, you choose it to meet new people and gain perspective, but the truth is that it can be a bit of a culture shock. I think that's all she's saying; it's a culture shock to her, and would be to many Canadians since most of us live in cities.
